    Immunization requirements

    7-12 grade:
    The Camden County Immunization Program offers free immunizations to eligible Camden County children infants through 18 years of age. Call 856.225.5128 for appointments.

    Physical Exams for Sports:  All athletic candidates must have a complete physical examination done within 365 days of the first day of practice in order to participate. It is preferred that the  physical examination may be performed by your child's private health care provider.  Dr. Stagliano, the school physician,  will offer sports physical exams if the student does not  have a medical home. Date and times for each sport will be in the announcements prior to the start of each season. The physical examination must be recorded on the school health history form.

    Developmental Stage Physicals:
    Haddon Heights School District requires all new students entering the district to have a physical exam. It is strongly recommended by the school district that parents obtain a physical exam at least once during each of the student's developmental stages. The developmental stages are defined as follows:
    * early childhood (preschool through grade 3)
    *pre-adolescence (grades 4 through 6)
    * adolescence (grades 7-12)

    Medication Administration:
    ECollect forms (health information and medical services) need to be filled out each school year in order for the School Nurse to administer over the counter medications and medical updates. 

    Mantoux Testing:
    Mantoux testing is required by the State of New Jersey on students who enter the Haddon Heights School District from areas of New Jersey and other states designated by New Jersey as high risk Tuberculosis areas. It is the responsibility of School Nurse to interpret, report results and secure information about the chest X-ray results and whether the private physician prescribed medication therapy.

    *Height and Weight screening annually,
    *Vision Screening in grades K, 2 , 4, 6, 8, 10 
    * Hearing Screening in grades K, 1, 2, 3, 4, 6, 8, 10, 
    * Scoliosis screening examination, which is a lateral curve of the spine most commonly found during the adolescent growth period, completed in grades 5, 7, 9, 11.
